State Rep. Karen Bennett to Hold ACA Special Enrollment Event

State Representative Karen Bennett of House District 94 will hold a special enrollment event on Sunday, November 10, 2013 from 7:30 AM to 10:45 PM at the Fairfield Baptist Church in Lithonia, Georgia. 

Navigators will be available at this event to help citizens enroll in an insurance plan using the federally-created insurance exchange system. Please bring items from the checklist below.

Marketplace Application Checklist:

  • Social Security Numbers (or document numbers for legal immigrants) 
  • Employer and income information for every member of your household who needs coverage (for example, from pay stubs or W-2 forms—Wage and Tax Statements) 
  • Policy numbers for any current health insurance plans covering members of your household 
  • A completed Employer Coverage Tool (Obtain form at healthcare.gov)
