Deficiencies cited in the report: Soap and/or paper towels weren't at the hand sink. A pot of stock was seen on the floor in the walk-in cooler.

Deficiencies cited: A visible light at the back door and weathering strips.

Name and location: Subway Sandwich Shop, 5160 Memorial Drive

Deficiencies cited in the report:  Area for employee belongings was being used inappropriately. Purses were seen above food equipment. 

APRIL 28

Deficiencies cited in the report: There was food debris and grease on food carts and trays. Single-service items weren't stored in wraps or container and were stored under a source of contamination. A restroom door latch was off track. 

APRIL 27

Name and Location: Little Caesars, 5998-A Memorial Drive

Score: 91/A

Previous scores: 94/A on Oct. 5, 2010, 95/A on Feb. 17, 2010.

Deficiencies cited in the report: Foods that could be hazardous weren't kept at temperatures of more than 135 degrees. Pepperoni pizza in a holding rack didn't have a timer on it.

Name and Location: The Original Pancake House, 5099 Memorial Drive

Score: 77/C

Deficiencies cited: Raw meats weren't separated from cooked and ready to eat foods in storage. Food that should be kept cold (fruit, sausage and corned beef) wasn't at a temperature of 41 degrees or below. The current inspection report wasn't posted. 

Name and location: Smoke Rise Country Club, 4900 Chedworth Drive

Score: 94/A

Previous scores: 93/A on Nov. 23, 2010 and 93/A on June 4, 2010

Deficiencies cited: The inside of a microwave was dirty, as were shelves in a walk-in cooler and a reach-in cooler. Utensils weren't stores on a clean portion of the food prep table. Drinks for employees were stored with food for service. 

APRIL 26

Name and location: Harowa Coffee, 5289 Memorial Drive

Score: 95/A

Previous scores: 96/A on April 14, 2011.

Deficiencies cited in the report: A faucet was missing from a three-compartment sink, and the person in charge was not able to sanitize food equipment.

Name and location: Woodridge Elementary School. 4120 Cedar Ridge Trail

Score: 90/A

Previous scores: 92/A on Nov. 18, 2010 and 82/B on April 30, 2010.

Deficiencies cited in the report: Moldy hamburger buns were seen. Utensils were stored wet. 

APRIL 25

Name and location: McDonalds, 919 North Hairston Road

Score: 71/C

Previous scores: 90/A on Feb. 4, 2010 and 79/C on Dec. 22, 2009

Deficiencies cited: An employee went from an activity that could soil or contaminate hands to food prep without washing hands. An employee was seen washing hands in a sink used for food prep.  There was no soap in the men's restroom. There wasn't any time documentation for controls on the sliced cheese and tomatoes. Employees were seen prepping food without the required hair restraints. A disposable cup with no handle was used to scoop ice. The inside of a walk-in cooler was rusting, as were shelves inside a cooler. There was condensation in a walk-in freezer. Cardboard was being used to line a floor. The inside of a walk-in freezer was dirty. There were leaks at two sinks. 

Name and location: Paccino's Pizzeria & Tasty Seasons, 4081 Redan Road

Score: 90/A

Previous scores: 70/C on Feb. 15, 2011 and 99/A on Oct. 13, 2010

Deficiencies cited:  Some foods in the prep cooler were not covered. Some cooked vegetables in were marked April 6, 2011 and "not in sound condition." Some bread in the freezer was past the expiration date of March 30. An ice scoop container wasn't cleaned and sanitized. 

Name and location: China Restaurant, 961 North Hairston Road

Score: 71/C

Previous scores: 85/B on April 6, 2010 and 88/B on Oct. 21, 2009

Deficiencies cited: Raw chicken was stored next to cooked chicken wings and shrimp. Uncooked chicken and beef weren't separated in storage. Food was uncovered in the coke cooler, reach-in cooler and walk-in cooler. The inside of a freezer wasn't clean. The shelves in the back, dry-food storage were dusty. Food was stored on the floor. Knives were stored with tools. Cardboard was used a liner on shelves. There was a leak at the three-compartment sink. 

ARPIL 20

Name and location: Wells Cargo Cafe, 953 Main Street

Score: 95/A

Previous scores: 92/A on Feb. 25, 2010 and 84/B on Aug. 28, 2009.

Deficiencies cited: Some out-of-date packaged foods hadn't been disposed of. Scoops were inside sugar, rice and Italian bread crumb bins. Some shelves inside a cooler were rusting. Aluminum foil was used as liner on a shelf. Some gaskets weren't clean inside the prep cooler. The outside of a freezer and refrigerator was unclean.
