April 12

Name and location: Magic Wok, 4857 Memorial Drive

Deficiencies cited in the report: Raw animal foods weren't kept separate from cooked foods or from foods eaten raw while in storage and during prep. Foods that should be kept cold were not maintained at a temperature below 41 degrees fahrenheit. 

 

Name and location: Three Stars Pizza & Wings, 4449 Rockbridge Road

Score: 88/B

Previous scores: 61/U on March 29, 2011; 76/C on March 8, 2011.

Deficiencies cited in the report: The restaurant didn't hire a certified food safety manager. Ready-to-eat food wasn't clearly marked with a date by which it should be eaten, sold or discarded. The restaurant didn't have storage for employees' personal items.

 

April 6

Name and location: The Wing Spot & Pizza, 5616 Redan Road

Score: 77/C

Previous scores: 84/B on Aug. 23, 2010; 80/B or March 24, 2010.

Deficiencies cited in the report: An employee was seen returning from the restroom without washing hands after entering the kitchen. There was a toothbrush at the hand sink. One employee was seen washing hands at a three-compartment sink. The soda nozzles were reported to be unclean. There were dishes in the vegetable sink and vegetables in the dish sink. Tables used for storage were rusty. Wood storage shelves weren't sealed with epoxy. Cooler exterior was cited as unclean. Garbage wasn't disposed of properly and smelled bad. Base boards needed repairing. Some employee personal items weren't stored in the designated area. 

April 4

Name and location: Hong Kong City, 4819 Rockbridge Road.

Score: 83/B

Previous scores: 59/U on March 22, 2011; 93/A on April 8, 2010.

Deficiencies cited in the report: Foods that should be kept cold weren't kept below 41 degrees fahrenheit. Bulk containers of salt, soy sauce and frying oil were stored on the floor. Bowls were seen being used as scoops. There was soil buildup on the walls, floors and toilet of the restrooms. Some employees' belongings weren't stored in the designated area. 

April 1

Name and location: The Village Cafe, 6670 Memorial Drive

Score: 96/A

Previous scores: 79/C on April 23, 2010; 91/A on Jan. 12, 2010

Deficiencies cited in the report: Chicken was seen thawing in water that wasn't running. The bottom of the prep table was rusting. 

 

Name and location: AmeriBistro, 945 North Hairston Road.

Score: 90/A

Previous scores: 90/A on Feb. 9, 2010; 73/C on Feb. 1, 2010.

Deficiencies cited in the report: Storage shelves for dry food in the back were unclean, as were shelves by the fryers. There was a bag of food on the floor in a rear storage area. An employee was seen handling food with artificial nails and polish. Cardboard was being used as a liner on prep tables and inside a cooler. The ceiling above the prep table didn't have proper tiles. The light in the women's restroom wasn't adequate.

March 31

Name and location: Echelon 3000 Ultra Bistro, 1179 South Hairston

Score: 96/A

Previous scores: 95/a on Aug. 20, 2010; 98/A on Jan. 21, 2010.

Deficiencies cited: Crawfish wasn't properly protected from a source of contamination. The chef was seen with jewelry other than a plain ring. The restroom door didn't have a self-closing device.

 

Name and location: The Original Pancake House, 5099 Memorial Drive.

Score: 72/C

Previous scores: 91/A on July 7, 2010; 85/B on Nov. 4, 2009.

Deficiencies cited: Employees were seen eating and/or drinking food in the prep area and or in the utensil storage or equipment areas. Raw animal foods weren't separated from cooked or foods eaten raw, and different types of meat weren't separated. Foods that should be kept cold weren't kept below 41 degrees fahrenheit. Foods that are kept hot weren't kept at temperatures of more than 135 degrees. Molded strawberries were seen in walk-in and reach-in coolers.
